Utility crashes when trying to Save Service Report

Every time I attempt to Save Service Report the Promise Utility crashes.

I am using a Pegasus2 R4, Mac Pro 2013 "trashcan" with MacOS (10.13.4) and have latest Promise firmware (5.04.0000.61) and latest Promise Utility (4.02.0000.10). The R4 file system is Mac OS Extended (journaled). I even reinstalled the latest firmware and utility just to be sure. Have tried rebooting entire system and tried removing power to the R4.

The reason I am trying to save service report is because I have seen 10 instances of "Command times out on physical disk" in the past 2 months. It is rarely the same disk. Each of the 4 disks have generated at least 2 of these errors but, it is always one disk at a time. No SMART errors. Disks are the original 2TB Toshiba (DT01ACA2) that came with the unit from Promise.

Any ideas?

UPDATE...

I was finally able to get Save Service Report to run without crashing the Promise Utility.

SOLUTION:

I completely uninstalled the Promise utility by going to the Applications folder and moving Promise Utility.app to the recycle bin. I also went to System Preferences / Users & Groups / Login Items and removed the 2 instances of PromiseUtilityDaemon that were there. (not sure why there was more than 1 instance). Next I rebooted my Mac Pro. Then I downloaded and installed the latest Promise Utility.

Now I can Save Service Report without crashing the Promise Utility. So far, no "Command times out on physical disk" but, time will tell.
